Load Testing Fall Protection Anchors: What the Standard Actually Requires
Load testing is one of those topics where the trade lore and the actual standard have drifted apart. Half the contractors we work with believe every anchor must be pull-tested every year. The other half believe that an installation test is enough for the life of the anchor. Neither is correct.
Here is what the governing standards actually require, written in plain language.
Installation proof loading — required, every time
CSA Z259.16 and ANSI Z359.6 both require that every newly installed engineered anchor be proof-loaded at installation to a value specified by the engineer of record. The default value is 1.25× the rated working load, applied for a minimum of 60 seconds, with no permanent deformation visible after the load is removed.
OSHA 1910.140 does not explicitly require an installation proof load, but in practice every reputable installer performs one because it is the only way to validate that the substrate (concrete, steel, wood) actually achieved the design pullout strength.
Recurring inspection — visual, not destructive
The annual inspection requirement under both CSA and OSHA is a visual and dimensional inspection by a qualified person, not a destructive load test. The qualified person looks for corrosion, deformation, cracked welds, loose hardware, substrate condition, and conformity with the original installation drawings.
Pull-testing every anchor every year is unnecessary, expensive, and — over many cycles — actually degrades the substrate. The standards know this; they specify destructive testing only at installation, after structural repair, and at engineer-determined recertification intervals.
When to add a load test to a recertification
The engineer of record will require a load test at recertification when:
- There has been a fall arrest event involving the anchor.
- There has been any modification to the substrate (re-roofing, repointing, structural repair).
- Visual inspection identifies corrosion or deformation that cannot be ruled out as cosmetic.
- The original installation documentation has been lost.
- The system has been in service for an extended period (typically 10+ years for static systems, 5+ for high-cycle systems).
How a proper load test is documented
A defensible load test report includes the calibrated load cell serial number and most recent calibration date, the applied load value and duration, photographic evidence of the test setup, before and after photos of the anchor, the qualified person's name and credentials, and the engineer of record's seal. Anything less is not a load test record — it is a story.
